Care Management, Power of Attorney and/or Guardianship Services (continuous services), $90.00/hour

• Care Manager Assistant Level I, $50.00/hour
• Care Manager Assistant Level II, $40.00/hour

Initial Assessments/Consultations, $125.00/hour

Legally-Related Work/Medical Record Reviews, $150/hour

Additional Off-Hour/Crisis Fees

• Evenings (after 6:00 pm), $25.00/hour
• Weekends, Holidays or any time for crisis intervention when the need is immediate, $60.00/hour
